THE ANTERIOR APPROACH FOR REMOVAL OF RUPTURED CERVICAL DISKK AND INTERBODY FUSION



Abstract
A 56 year old farmer had incomplete Brown-Sequard syndrome below the level of Th-4, and bilateral lower cervical nerve root signs as sequel of his16 month old injury to the neck.
Plane X-ray studies showed mild kyphosis, narrowing and osteophyte at the interspace of C_(5-6). The discogram revealed a disc herniation toward theright at the same interspace. The additional ervical myelogram, however, missed to demonstrate this abnormality.
The disc and osteophyte were removed successfully and the inter body fusion, using iliac bone graft was carriea out by means of the anterior approach technique described by Cloward in 1958.
The instruments used in this case were modified from Cloward¢¥s original one soling the adjustable depth gauze to the upper half portion of the drill in lieu of puling them on the top of the guard. Also Galt skull trephine was used and which found to be convenient to remove bone graft from the iliac Plate for the inter body fusion.
